Comments:

On 7/29/2019 one inspector conducted a renewal study (10:40am to 1:35pm). 28 participants were in care with seven direct care staff with the residents (ratio - 1:4). Four participant records and three staff records were fully reviewed, and new staff were included in the sample. A physical plant tour was done, the medication area was checked; staff and participants were interviewed. The facility staff requested a training session in assessments and plans of care, and this was scheduled for 8/5/2019.

Description:	Based on document review, the facility failed to address an assessed need on a plan of care (POC). EVIDENCE: 1. The initial assessment for participant 2, dated 7/16/2019, shows a need for assistance with bathing. This is not addressed on the POC, dated 7/16/2019.
Plan of Correction:	EADC will correct this particular Plan of Care, review and correct al other POC's and have scheduled training for POC's procedures & guidelines.
