Comments:
An unannounced non-mandated inspection was conducted on 4/12/19 by 1 licensing inspector from approximately 11:00 a.m. - 12:45 p.m., in response to a reported complaint. During the inspection, a tour of the facility was conducted, 1 resident record was reviewed, 1 staff record was reviewed, facility documentation was reviewed, menu calendar was reviewed, food supply was checked, lunch meal was observed and interviews of residents and staff were conducted. The facility was found to be in compliance with the allegations reported in complaint. There was a non-complaint related violation cited during the inspection. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to the Inspector within 10 days. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on observation during the tour of facility, the facility failed to ensure that bed linens are changed at least every seven days and more often if needed. Evidence: There was dry blood present on resident #1 pillow case.

Plan of Correction: Bed linen was changed by direct care staff (DCA). The DCA was in the room before the room was cited. The room was due for linen change next. In the future, aides will change linen as early in the day as possible.
