Comments:

On 04/30/2019 two VDSS inspectors were on site responding to a complaint made against the facility. During the entrance interview the facility Administrator was made aware of the purpose of the inspection. Facility records and interviews were conducted. Based on the documentation submitted and interviews conducted there is not substantial evidence to validate the complaint. However, the noncompliance was revealed during this complaint investigation is contained within this report. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to the Inspector within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description:	Based on the review of facility records with the facility Administrator the facility failed to ensure that at the time the medication is administered, the facility documented on a medication administration record (MAR) all medications that were administered to residents, including over-the-counter medications and dietary supplements. Evidence: Facility MARs charting for March and April 2019, that were submitted for the inspectors review noted various days that facility staff did not indicate whether medications or treatments were administered to the resident. For example, (1)03/25,28/2019, facility staff did not document whether the resident's blood sugar level was checked. 03/01,09,10,14,15,20,25/2019 facility staff did not document whether the resident was administered the medication Fiber-Lax as prescribed. (2)-For the month of April 2019 facility staff did not document on the MAR whether the medication Fiber-Lax was administered on 04/01,04,11,20,26,30/2019. Upon request the facility did not submit for the inspectors review documentation that indicated why these medications were not administered.
