Comments:
An unannounced monitoring inspection was conducted on this date from 9:45a.m. until 11:45a.m. with coworker W.B. The administrator, manager, and the facility consultant were present during the inspection. A resident record and the facility's shift log were reviewed. According to the documentation the inspectors reviewed during the inspection, a resident was discharged from a mental health hospital on 6-5-19 and admitted to the facility on the same day, on what the administrator called "a 30 day trial ". The inspectors reviewed the shift report documenting that same resident walked away from the facility on Sunday night(6-9-19) , at approximately 9:30p.m. No documentation of the resident signing out was observed on file. The manager said the resident did not tell anyone he was leaving . The manager stated during the afternoon of 6-11-19, the resident was found by the police and due to suicidal ideation was taken to a hospital and the hospital staff notified the mental health agency. The manager also told the inspectors, the agency staff notified the facility later that afternoon to let them know the resident had been found and was in the hospital. The administrator stated on the afternoon of 6-11-19, there were numerous telephone conversations between the facility staff and the agency staff regarding the resident's return to the facility. The facility administrator decided the resident would not be able to return to the facility because the facility could not provide safety for a resident who refused to stay on site . The agency staff was told on 6-11-19 via telephone that the resident was discharged from the facility . The facility's written discharge notice was sent by fax on 6-12-19 to the agency staff and licensing . While on site the inspector asked about the delay in response to e-mail sent on 6-10-19@8:01p.m, regrading the resident who wandered away . The manager stated she had just received the licensing staff e-mail sent on 6-9-19, the morning of the inspection (6-12-19). The administrator said he would have the computer checked to try to determine why there was a delay in receipt of the e-mail. Also the administrator shared he recently purchased a new ice maker , a new hot water tank, and a new AC system. He also added remodeling of the elevator to meet current code requirements was in progress.
