A complaint investigation was conducted on 5/27/19 and concluded 6/4/19. The complaint alleged that the facility failed to give a resident a 30 day notice prior to discharging him. Based on a review of written and photographic documentation and interviews with the facility staff it was determined that the complaint was not valid.
