Comments:
An unannounced focused monitoring inspection was conducted on 7/11/19 (8:25 AM - 11:30 AM) to follow-up on a high-risk violation that was cited on 5/22/19. Medication administration was observed and facility documents were reviewed. The violation was discussed and an exit meeting was held. Areas of non-compliance are identified on the violation notice. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return to the licensing office within 10 calendar days. Please specify how the deficient practice will be or has been corrected. Just writing the word 'corrected' is not acceptable. The 'plan of correction' must contain: 1) Steps to correct the non-compliance with the standards, 2) Measures to prevent the non-compliance from occurring again, and 3) Person responsible for implementing each step and/or monitoring any preventative measures. Standard #: 22VAC40-73-650-E
Description: Based on record review, the facility failed to ensure that the resident's record contains the physician's or other prescriber's signed written order or a dated notation of the physician's or other prescriber's oral order. Evidence: Medication administration, for Resident #1, was observed during the inspection. Resident #1's record contained a list of the resident's medications, but the record did not contain the signed written orders for the resident's medications.

Plan of Correction: The resident record did contain a physical with medication listed without dosage, directions or diagnosis the RCC requested clarification upon admission and a separate list was sent from the physician that did not have a signature. The resident receives all medication from Kaiser that is sent to the community then sent to the pharmacy for repacking. The RCC obtained a signed physician order on 7/11/2019. The RCC and ED have added the signed physician order to the admission checklist.
