Comments:

On 8/21/2019 an investigation was begun (times not recorded) online regarding a self-reported situation. The administrator let her ALF Administrator license expire. It was later reported that the license was renewed, and this was verified online on 8/27/2019 (times not recorded).

Description:	Based on interview and document review, the facility, which is licensed for both residential and assisted living care, failed to have a licensed assisted living facility administrator. EVIDENCE: 1. A phone interview with the owner revealed that the administrator let her license expire as of 3/31/2019. This was confirmed by looking it up on line at www.dhp.virginia.gov. A call from the owner later in the day revealed that the administrator renewed her license. This was verified on 8/27/2019 by looking it up at www.dhp.virginia.gov.
