Comments:

On 10/2/2019 one inspector conducted an investigation into self-reported incidents (3:05pm to 4:25pm). Five resident records were partially reviewed, and eight staff records were partially reviewed.

Standard #:	22VAC40-73-440-F
Description:	Based on documentation review and interview, the facility failed to ensure that a uniform assessment instrument (UAI) was updated when a resident's condition changed. EVIDENCE: 1. Resident 1 developed agitated/aggressive behavior when the medications were changed, according to progress notes in the resident record, and interview with facility staff. The private pay UAI, dated 4/29/2019, was not updated to reflect the new behaviors.
Plan of Correction:	The UAI for resident #1 to be updated by the Administrator to accurately reflect the resident?s care needs. Administrator and/or Healthcare Coordinator to ensure the UAI is updated in a timely manner when a resident?s condition changes.
