Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. renewal inspection was initiated on 06/26/2020 and concluded on 06/29/2020. The Staff Person in Charge was contacted by telephone to initiate the inspection. The Assistant Administrator reported that the current. The inspector emailed the Assistant Administrator a list of items required to complete the inspection. The inspector reviewed 4 resident records, 4 staff records and additional records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-73-650-C
Description: Based on record review the facility failed to ensure Physician's or other prescribers orders oral orders shall be reviewed and signed by a physician or other prescriber within 14 days.

Evidence: On 06/26/2020 while reviewing the physician orders sheet for resident #4, the "current orders as of October 2, 2019" provided for resident #1 was not signed by the physician within in 14 days.

Plan of Correction: LPNs and RN will check MARS weekly for two months, and then monthly to ensure all orders are signed. Other med aides and nurses will also monitor orders for signatures from physicians. Med aides and nurses will fax the MARS back to the doctor to ensure that they will sign all MARS. The LPN will report to the Asst. Administrator who will ensure that physicians? orders have met state regulations regarding physicians?

Description: Based on resident record review the facility failed to ensure medications shall be administered in accordance with the physician's or other prescriber's instructions and consistent with the standards of practice outlined in the current registered medication aide curriculum approved by the Virginia Board of Nursing.

Evidence #1: On 06/26/2020 while reviewing the medication administration record for resident #4,noted under the exceptions for resident #4 are a total of 199 resident refusals for the following medications: Benztroping mes 0.5mg tab, Phenazopyidine 200mg tab, Levetiracetam 750mg tab, Risperidone 4mg tablet, Vitamin B6 250mg tab, Amlodipine 10mg tab, Haloperidol 2mg tablet, Doxazosin Mesylate 1mg tab, Lisinopril HCTZ, Rapaflo 8mg capsule, Omprazole DR 40 mg capsule.

Evidence #2: On 06/26/2020 while reviewing the medication administration record for resident #1, under the "Exceptions for resident #1, Reason", states " Awaiting Pharmacy" for Drysol Solutions for the following date: 05/01/2020, 05/02/2020, 05/03/2020, 05/06/2020, 05/07/2020, 05/08/2020, 05/09/2020, 05/10/2020, 05/13/2020.

Plan of Correction: LPN will check the orders weekly for two months, and then monthly (to prevent). Other med aides, LPNs and R.N. will also report to the asst. administrator about the status of physicians? orders, if the orders are needed or not. LPN will report weekly to the Asst. Administrator about his findings and other medical staffs? findings about the status of physicians? order. All med aides will give copies of all orders to the LPN, and have staff file them in the orders book and in the residents? files. Training was conducted with nurses and med aides regarding refusals of meds by the Asst. Administrator, Med Trainer. They signed the information and also signed refusal forms that they must now fax to the doctor.
