Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was initiated on 06/18/2020 and concluded on 07/15/2020. A complaint was received by the department regarding allegations in the areas of resident care. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation. The evidence gathered during the investigation did not support the allegations of non-compliance with standards or law. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Description:	Based on resident record review and interview, the facility failed to complete a uniform assessment instrument (UAI) in accordance with Assessment in Assisted Living Facilities (22VAC30-110). EVIDENCE: 1. The UAI for resident 1 dated 2/25/2020 shows this resident needs help with bathing at the level of Performed by Others. In an interview with the administrator it was discovered that the resident requires physical assistance with bathing, and this is also confirmed on the individualized service plan (ISP).
Plan of Correction:	Administrator inadvertently checked the wrong box. This has now been corrected and all UAIs will be reviewed carefully in the future.
