Comments:
This inspections was conducted by licensing staff using an alternate remote protocal due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was inititated on 05/22/2020 and concluded on 08/12/2020. A complaint was recieved by the department regarding allegations in the areas of 22VAC40-74-(4) STAFFING AND SUPERVISION, 22VAC40-73-(7) RESIDENT ACCOMODATIONS AND RELATED PROVISIONS, 22VAC40-73-(6) RESIDENT CARE AND RELATED SERVICES, 22VAC40-73-(2) ADMINISTRATION AND ADMINISTRATIVE SERVICES. The Administrator was contacted by telephone to conduct the investigation. The evidence gathered during the investigation supported the allegation(s) of non-compliance with standards or law, and violations were issued. Any violationsnot related to the complaint but identified during the course of investigation can be found on the violation notice.

Standard #: 22VAC40-73-70-A
Complaint related: Yes
Description: Based on record review the facility failed to report to the regional licesing office within 24 hours any major incident that has negatively affected or that threatens the life, health, safety or welfare of any resident.

Evidence #1:On 08/12/2020 while reviewing the facility, "Incident report (For internal use only)" fall, dated 02/21/2020 for resident #1, documentation reads, "Aide made rounds on resident. Went in room, resident on floor Rt. side. Sm, cut that was bleeding over Rt eyebrow. Area cleaned and sm.dsg applied. Resident also has a skin tear to left lower arm.also clean and bandage applied. Alert and talking. Assisted back to bed".

Evidence #2: The facily did not report to the regional licensing office within 24 hours of the incident that occured on 02/21/2020 that negatively affected or threatened the life, health, safety, or welfare of resident #1.

Plan of Correction: Adminisrtator / DON will ensure inident reports will be sent within 24 hours of the incident to the Licensing Office of any major incident.

Complaint related: Yes
Description: The facility failed to ensure the medicationadministration record contained any errors or omissions.

Evidence: On 08/12/2020 while reviewing resident #1's, medication administration record which was blank and did not document any errors nor ommisions for January 5, 2020 and January 27, 2020 for the following medications: Potassium ER 10 MEQ Tablet, Docusate Sodium-Senna Tablet and Bumetanide 2MG Tablet.

Plan of Correction: DON will run a daily variance report to detect any errors or omissions.
