Technical Assistance:
1) Ensure staff B completes the required dementia training hours by 4/30/21.
2) Ensure initial physicals are reviewed for all allergic reactions when multiple allergies are listed.
3) Recommended attaching health care oversight notes/issues from records reviewed to the health care oversight form.
4) Even though a staff in charge is noted on each unit and on the schedule, recommended adding a notation for the one person who is designated in charge of the facility.
5) Even though it is noted on the individualized service plan for Gardner Wing residents that the legal representative received a copy, recommended the legal representative sign their initials next to this statement as well.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 3/22/21 and concluded on 3/24/21. The director of assisted living was contacted by telephone to initiate the inspection. The director of assisted living reported that the current census was 64. The inspector emailed the director of assisted living a list of items required to complete the inspection. The inspector reviewed four resident and four staff records. Selected sections of two additional resident and four additional staff records were also reviewed. The activities calendar, menu, staff schedules, fire drills, dietary/medication/health care oversight reviews, January and February medication administration records, physicians' orders, all criminal record reports for all current staff hired since the last inspection, as well as other documents were reviewed. A virtual inspection and tour were also conducted. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
