Technical Assistance:

Technical assistance provided regarding the annual health inspection as it relates to standard 22VAC40-73-580 A concerning initial and subsequent annual reports from the Virginia Department of Health. The report shall be retained at the facility for a period of at least two years.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 1/19/2021 and concluded on 1/23/2021. The licensee/owner was contacted by telephone to initiate the inspection. The licensee reported that the current census was eight (8). The inspector emailed the licensee/owner a list of items required to complete the inspection. The inspector reviewed four (4) resident records/, four (4) staff records, resident , notes, MARS, U.A.I. physician orders, resident rights, healthcare oversight, fire inspection, I.S.P.s, admission examinations, TB screenings, staff trainings, and certifications submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	VIOLATION: Based upon the documents submitted by the facility, the facility failed to include a comprehensive individualized service plan for each resident in care. EVIDENCE: The documents recieved from the facility on 01/23/2021 failed to have a comprehensive individuualized plan for resident #2.
