Comments:

On 11/9/2021 a complaint (54848) investigation was done from 9:20 am to 10:45 am regarding resident care. One resident record was reviewed and staff members were interviewed. Based on available evidence, the complaint is not valid. Violations noted that are not complaint related are on the violation notice.

Description:	Based on resident record review, the facility failed to obtain a written agreement between the resident or legal representative and the facility at or prior to the time of admission. EVIDENCE: 1. Resident 1 moved in on 10/25/2021 and the resident agreement was obtained on 10/27/2021.
Plan of Correction:	This resident was dropped off and left prior to all arrangements being finalized. Normally this document is signed on the day of admission, and the guardian did not send it back in time. In the future, admission will be refused until the resident agreement is signed and obtained.

Description:	Based on resident record review, the facility failed to have either a preliminary plan of care (POC) or a comprehensive individualized service plan (ISP) completed by the day of admission. EVIDENCE: 1. Resident 1 moved in on 10/25/2021. A POC was not completed, and the ISP was done on 10/31/2021.
Plan of Correction:	From now on a preliminary plan of care will be done within seven days prior to admission or on the day of admission, and the comprehensive ISP will be completed within 30 days of moving in, or, a comprehensive ISP will be completed will be completed on the day of admission.
