Comments:
Unannounced complaint inspections were conducted on 6/30/23 and 7/13/23; in response to a complaint that was received by the licensing office on 6/5/23 regarding: Resident Care and Related Services. Interviews were conducted and facility documentation was observed. The violation was discussed and an exit meeting was held.

The evidence gathered during the investigation did not support the allegation of non-compliance with standard(s) or law. However, violation(s) not related to the complaint but identified during the course of the investigation can be found on the violation notice. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Complaint related: No
Description: Based on record review, the facility failed to ensure that the rights and responsibilities of residents are reviewed annually with each staff person.
Evidence: The last review of the rights and responsibilities of residents in assisted living facilities, documented for Staff #1, was completed in January 2022. The review was more than a year old, at the time of the inspection.

Plan of Correction: Residents' Rights and Responsibilities will be reviewed with each staff member by or during the next all-staff meeting.

The Executive Director will collaborate with HR in performing an all-staff file audit to identify and rectify, if needed, similar situations with all staff.

The Executive Director will collaborate with HR to ensure that Resident's Rights and Responsibilties annual review and training is covered under scheduled training in the community either online or in person.

Complaint related: No
Description: Based on observation; the facility failed to ensure that medications remain in their original containers, labeled with the resident's name, or in a pharmacy-issued container, until administered.
Evidence: Resident #1's morning medication was observed during the inspection. Resident #1's Cholecalciferol and Cyanocobalamin were not labeled with the resident's name, at the time of the medication cart inspection.

Plan of Correction: Resident #1's Cholecalciferol and Cyanocobalamin have been labeled with the resident's name.

DON and ADON will perform med cart audits to identify and rectify, as needed, similar concerns.

Refresher in-service will be provided for MedTech and Licensed Practical Nurses on the proper procedure for labeling family provided medication.
