Today's unannounced focused monitoring inspection was completed on 07/10/2019 from 11:05 A.M.-3:00 P.M. The purpose of the inspection was for determining compliance to previous violations. There were 32 children in attendance with seven staff, and one administrator present. The records for three staff were reviewed, two children records, parent handbook and staff handbook, and classroom observations. The children were observed during one-on one interactions, behavioral guidance, free choice activities that included blocks, table top activities, and dramatic play, story time, and STEM activities. Children were also observed during transition to lunch, handwashing, lunchtime, and naptime. Exceptions to compliance are noted on the violation notice. Standard #: 22VAC40-185-240-B Description: Based on a review of the center's written policies and procedures and interviews with administrative staff, staff were not provided in writing with all required information by the end of the first day of supervising children. Evidence: 1. Documentation provided to staff did not contain the following required information; procedures for supervising a child arriving after scheduled classes or activities. 2.Administrative staff verified the required policies were not included in the written information that employees are provided upon hire. Plan of Correction: Administration will update the handbook with the required policy and distribute to all staff members.
Standard #: 22VAC40-185-550-M Description: Based on review of injury reports, the center failed to include all required information on the injury report. Evidence: 1. Injury report dated 07/02/2019 was missing the time the parent was notified. 2. Injury report dated 07/03/2019 was missing the time the parent was notified. 3. Staff verified that the time parents were notified was missing from the report. Plan of Correction: Administration will review accident/injury form,and the required information that needs to be on the report with all staff.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of staff records, the center failed to obtain a central registry finding within 30 days of employment. Evidence: 1. The record for Staff #3 did not contain a record of a central registry finding. Staff #3's day of hire was 01/28/2019. 2. Staff verified that the Central Registry check has been completed, but had not received the background check back, and there had been no follow-up activity in regards to the background check. Plan of Correction: Administration immediately reached out to the Office of Background Information requesting information on the status of the central registry check.
