Comments:
An announced initial inspection was conducted on August 21, 2019, from approximately 8:10 AM to 10:45 AM. Space and equipment observed were appropriate for school age children. The inspection will be reviewed by administrator for license issuance.

Violations:
Standard #: 22VAC40-185-40-I
Description: Based on observation , the licensee did not have written procedures for injury prevention. Evidence: The center did not have written procedures for injury prevention plan.

Plan of Correction: The center will develop injury prevention procedures.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the applicant did not ensure the areas ad equipment of the center, inside was maintained in a clean, safe and operable condition. Evidence: In the girl's restroom , the two stall dividers is rusting and a red residue comes off the divider.

Plan of Correction: The church will sand and repaint divider.

Description: Based on record review, the licensee did not ensure the emergency preparedness plan contain procedural components. Evidence: The following emergency preparedness plan procedural components were missing : 1- Sounding of alarms 2- Emergency communication - Availability and primary use of communication 3- Evacuation procedures including head counts specific assembly points for the center, securing essential documents and special health care supplies to be carried off site, and methods of communication after the evacuation. 4- Shelter in place procedures including scenario applicability, inside assembly points, head counts, primary and secondary means of access and egress, securing essential documents (sign-in records, parent contact information, etc.) and special health supplies to be carried into the designated assembly points and methods of communication after the evacuation. 5- Staff training requirement, drill frequency, and plan review and update 6-Other special procedures developed with local authorities

Description: Based on observation, the applicant did not ensure emergency evacuation and shelter in place maps and procedures were posted in a location conspicuous to staff and children . Evidence: The director confirmed the center did not have posted the shelter place map and emergency procedures.

Plan of Correction: The center will have the procedures and maps posted.

Standard #: 22VAC40-185-550-F
Description: Based on observation, the licensee did not ensure a 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center will be posted in a visible place at each telephone. Evidence: The emergency numbers were not posted in a visible place.

Plan of Correction: The emergency numbers will be posted.
