Standard #: 22VAC40-185-70-A Description: Based on record review and discussion with the staff, the facility failed to ensure that the staff records shall contain all of the elements as required by the standards.
Evidence: The record for Staff #4 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment; the record only contained one documented reference.Plan of Correction: The reference will be completed by 09/30/2019.
