An unannounced monitoring inspection was completed on 9/17/19. There were 26 children present with two staff supervising. Three children's records and two staff records were reviewed. Two emergency medications were at the center for one child in care.
The inspector arrived at the school at 2:15 pm and departed at 4:40 pm.
The children were observed at arrival, during free table toy time, during snack, and during free time.

Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that all required information was documented in each child's record.
Evidence:
1. There were no work phone numbers for the mother or father of child #1. There were work places documented in the child's record.
2. The record for child #3 did not have a first date of attendance documented.Plan of Correction: Staff will have all information documented in the child's electronic file.
Standard #: 22VAC40-185-510-E Description: Based on observation, the center failed to ensure that medication was labeled with the child's name, time or times to be given and dosage amount.
Evidence:
1. There was a Benadryl liquid antihistamine, unopened, in a ziploc bag with the child's name. The medication was not labeled with the child's name, time or times to be given, and dosage amount.Plan of Correction: Staff will label the medication with all required information.
Standard #: 22VAC40-185-510-G Description: Based on observation, the center failed to ensure that a prescription medication was maintained in the original, labeled container from the pharmacy.
Evidence:
1. There was an Epipen observed in a plastic ziploc bag. The bag had the child's name on it. The Epipen was not labeled in any way and did not have the pharmacy label or original labeled container. The Epipen was in the clear plastic capped container with the Epipen inside.Plan of Correction: Staff will ask the parent to provide the original box with the pharmacy label attached.
