Comments:
An unannounced monitoring inspection was completed on 9/26/19. There were 74 children present in nine groupings. Seven children's records and seven staff records were reviewed. Five emergency medications for two children in care were observed.

The inspector arrived at the center at 9:40 am and departed at 2:30 pm.

The children were observed during art activities, during story times, during outside times, during a field trip, during large group time, during free play times in the classrooms, during lunch, and during nap.

There was discussion with the director about the following: use of the mobile application for various tasks and communications, new subsidy requirements (memo dated October 2018), requirements for the upcoming renewal inspection including the new application and attachments required, medications, supervision during nap.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. There were three cleaning substances observed unlocked on a shelf in a two year old classroom bathroom. The substances were two canisters of disinfecting wipes and a disinfectant spray. The shelf where the substances were located was approximately six feet high and the substances were out of the reach and inaccessible to the children.

Plan of Correction: This was corrected during the inspection. The supplies were locked in the classroom's locked closet.

Standard #: 22VAC40-185-280-G
Description: Based on observation and interview, the center failed to ensure that hazardous substances not kept in original containers were in substitute containers that were clearly marked indicating their contents.

Evidence:
1. There were two generic, clear spray bottles observed locked in a cabinet in the diapering area of one toddler classroom. The two bottles were not labeled to indicate the contents. Staff stated that one was soap and water and one was bleach and water.

Plan of Correction: This was corrected during the inspection. The spray bottles were labeled with the contents.

Description: Based on observation and measurement, the center failed to ensure that the hot water in sinks used by the children was less than 120 degrees.

Evidence:
1. The water in two sinks was observed to the touch to be very hot. A digital thermometer was used to measure the temperature of the water in the two sinks. One sink's hot water measured 122.6 degrees Fahrenheit and one measured 124.6 degrees Fahrenheit. Both sinks were in a toddler classroom.

Plan of Correction: The director called the plumber to reset the hot water temperature. The director stated that new hot water heater was installed recently.

Description: Based on observation and interview, the center failed to ensure that the ratio of children from birth to 16 months was maintained at all times.

Evidence:
1. The infant room was observed at approximately 1:25 pm. There were 13 infants in cribs and three staff present. The children were 2 months through 13 months. One child was observed asleep on a cot, nine infants were observed asleep in cribs, one infant was observed being held and rocked, and two infants were observed awake in their cribs.

2. According to staff interviews, the ratio is doubled from approximately 12:30-2:30 daily when two staff take breaks for one hour each.

Plan of Correction: The director will have a staff person cover the breaks.

Standard #: 22VAC40-185-500-C
Description: Based on observation and interview, the center failed to ensure that toilet chairs were empited promptly and cleaned and sanitized after each use.

Evidence:
1. There was a toilet chair observed in the classroom bathroom of a two year old room. The chair was approximately 3/4 full of liquid. Staff #1 was asked about the contents and stated that the children use this sometimes without the staff's knowledge and that a child had apparently used it. The inspector was in the class for approximately 10 minutes and noticed the toilet chair at arrival into the classroom.

Plan of Correction: This was corrected during the inspection. The staff person emptied, cleaned and sanitized the toilet chair after the inspector asked about the chair being full.

Standard #: 22VAC40-185-550-F
Description: Based on observation, the center failed to ensure that 911 and the regional poison control phone numbers were posted at each telephone.

Evidence:
1. There were two phones observed in classrooms that did not have the required emergency numbers posted at the phones.

Plan of Correction: The numbers will be posted today at each phone.
