The inspector conducted an unannounced non-mandated unannounced inspection due to a complaint received in this office 10/07/2019. Records were reviewed, staff and a child were interviewed, and video reviewed. Child and staff records were reviewed. based on the evidence gathered, there is preponderate of evidence so the complaint is determined to be substantiated. Standard #: 22VAC40-185-340-F Complaint related: No Description: Based on interviews, and review of video, the center did not always have sight and sound supervision for children under 10 years of age.
Evidence:
1. The inspector reviewed a center video where staff #1 opened the door to the outside, and child #1 walked behind the teacher and went outside. 2. Staff #2 was out taking trash outside, and staff #3 who was on the playground noticed child #1 on the sidewalk and notified staff #2 who ran and got the child and took the child back inside to the classroom. The child was left unsupervised for approximately 3 minutes. 3. Staff #1 stated that "I never saw her go out the door."Plan of Correction: 1. Center staff consulted with the fire chief and building maintenance about the door lock. The door will now only be used as an emergency exit only. The administrator placed the door alarm to alert if the door is opened and emergency use only sign. Staff was trained to only take the trash out at the very end of they day using the exit doors that enter the playground area only. Staff received additional training on keeping children within sight and sound supervision.
