An unannounced monitoring Code compliance inspection was conducted today, 8/28/19. A review of staff and children's records was also conducted. There are 16 children in care ranging in age from 4 months to 2 years of age with five staff present. Upon arrival the children were outside strolling through the parking lot and playing on the playground. Lunch and diaper changing was also observed. We discussed fingerprint background checks and central registry checks. Should you have any questions do not hesitate to contact me. Inspection began: 10:35 am ended 1:45 pm.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of staff records the center failed to have all staff complete a central registry check. Evidence: 1. Staff # 1-8 do not have a current central registry (Child Protective Services) check on file. All staff have been working more than 30 days. Plan of Correction: The director will ensure that all staff submit to central registry background checks.
Standard #: 63.2(17)-1716-A Description: Based on observation the center failed to have posted in a visible location that the center is exempt from licensure. Evidence: 1. The center does not have posted the fact that they are exempt from licensure Plan of Correction: The director will this post this information.
Standard #: 63.2(17)-1716-A-6 Description: Based on observations and interviews the center failed to have all the required written information for the parents or guardians. Evidence: 1. The written information provided to the parents/guardians did not include information of enrollment capacity or public liability insurance coverage. Plan of Correction: The director will provide written information covering these areas.
Standard #: 63.2(17)-1716-A-9 Description: Based on observation the center was not following all safe sleep recommendations. Evidence: 1. The cribs being used by the infants had bumper pads inside. Plan of Correction: The director will remove the bumper pads.
Standard #: 63.2(17)-1716-B-5 Description: Based on observation of the playground area the center failed to ensure that the playground equipment had cushioning materials under them. Evidence: 1. The fenced off playground area was on asphalt. There were two pieces of Little Tykes climbing equipment in this area with no cushioning material under them. Plan of Correction: The director will discuss with the board what they want to do with this area to provide cushioning materials.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of staff records the center failed to have all staff submit to a fingerprint-based criminal history check. Evidence: 1. Staff #1-9 do not have a fingerprint based criminal history check on file. All staff are currently working in the classrooms. Plan of Correction: The director will ensure that all staff submit to fingerprint background checks.
