Technical Assistance:
It was discussed that the provider has not been operating since the end of November 2018 and just began services again on Tuesday, 5/28/19. The renewal process was discussed and the provider was advised to submit the renewal application and required materials as soon as possible.

Comments:
An unannounced monitoring inspection was conducted between the hours of 8:10am through 10:00am. There were a total of 4 children present (9 points), ages 2 months through 5 years with the Provider. A total of 4 children?s files, the provider's file and one pet file were reviewed. The children were engaged in free choice play and reading. Infant care and feeding and hand washing procedures were observed. Positive interactions between the provider and children were observed during this inspection. A complete inspection of the programming space, children and staff records, evacuation drills, menu, attendance, and emergency procedures were completed today. The provider advised there was no medication on site today. There was an adequate supply of age appropriate materials and toys. Description: Based on record review, 4 of 4 children's records reviewed were incomplete. Evidence: 1. Child #1's record was missing the medical insurance policy number and the parents' annual review of emergency contact information. Child #1's last review of emergency contact information was completed on 1/6/2017. 2. Child #2's record was missing the medical insurance policy number. 3. Child #3's record was missing the parents' annual review of emergency contact information. Child #1's last review of emergency contact information was completed on 9/19/2016. 4. Child #4's record was missing the medical insurance policy number, immunization record and authorization for emergency medical care.

Plan of Correction: I will get all things from the parents.

Standard #: 22VAC40-111-70-A
Description: Based on record review, 4 of 4 children's records did not contain documentation that parents were provided written information before the child's first day of attendance. Evidence: 1. Child #1, Child #2, Child #3, and Child #4's records do not contain documentation the parents were provided with the family day home's policy for the administration of medications.

Plan of Correction: I will provide this information to the parents and I will have them sign saying they received it.

Standard #: 22VAC40-111-240-A
Description: Based on observation, an area of the family day home has not been maintained in a clean and safe condition. Evidence: 1. Peeling paint was observed from the floor to ceiling on the corner wall behind the toilet in the restroom used by children in care.

Plan of Correction: I will scrape the corner and repaint.
