Comments:
An unannounced monitoring inspection was conducted today 6/3/2019. The inspection began at 10:40am and concluded at 2:30pm. There were 55 children present with 13 direct care staff and the Program Director were present. Seven staff's records and Six children's records were reviewed. During the inspection all eight classrooms in operation during the inspection were viewed, play time inside and outside was observed, lunch time was observed, infant care was observed, nap time and emergency supplies were viewed. Discussion with program director about: renewal application, measuring for capacity, new outdoor play space and insect repellent requirements.

Violations:
Standard #: 22VAC40-185-280-D
Description: Based on observation of the infant room, the center failed to ensure that all chemicals were not stored with or above food or single service equipment. Evidence: 1. Observed two spray cans of chemicals stored with grahmn crackers, suckers and candy.

Plan of Correction: The chemicals will be stored on the lower shelf.

Standard #: 22VAC40-185-340-F
Description: Based on observation at arrival to the program, the center failed to ensure that all children under the age of 10 years were in actual sight and sound supervision of a staff member. Evidence: 1. Observed two groupings of children on the black top receiving water to drink. The LI was at the end of the lot observing the two groupings of children. The age of the children were from 16 months to 3 years of age. Two children from the grouping of children were running away from to behind the vans that were parked at the edge of the lot. The two children were out of sight supervision for approximately 2 to 3 minutes. The vans were at the opposite end of where the LI was observing from. One of the teachers was observed sitting on the black top. The other four teachers were gathering and discarding cups and preparing to line the children up to enter the building.

Plan of Correction: The director will speak with the teachers. Normally on a Monday the vans are moved to the lower lot to ensure that they are not a hazard to the children. They were not moved today. In the future the vans will be moved ASAP on Mondays.

Standard #: 22VAC40-185-560-F
Description: Based observation of lunch time, the center failed to ensure that choked hazards are not served to children age three and under. Evidence: 1. Observed in the toddler room, they were served cut up pieces of hotdogs. The pieces were approximately 1/4 inch thick and posed a choke hazard to the children in care. The youngest child in the grouping was 12 months old. 2. Observed in the two year old classroom, they were served hot dogs on a bun. The hot dog was cut down the middle. This posed a choke hazard. The youngest child in the grouping was 16 months. 3. Observed in the 2 1/2 to 3 year old classroom, they were served whole hotdogs on a bun. This posed a choking hazard to the children.

Plan of Correction: The cook and the director will work to ensure that hotdogs are not served as a choke hazard again. The menu will be revamped to included non choke hazards.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of seven staff's records, the center failed to ensure that all staff had the finger print eligibility letter returned prior to starting work. Evidence: 1. Staff # 3 started on 2/25/2019 and the finger print eligibility letter was returned on 3/5/2019.

Plan of Correction: The director stated that there were staffing circumstances that occurred that required a staff person immediately. This is not the normal practice, but was necessary due to a staff person going into labor. In the future all timeframes will be followed.
