Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on 11/15/19 from. to 3:00 p.m. to 5:00 p.m.
At the time of entrance the following adult to child ratios were observed:
5 children, ages 4 to 5 years old, with one staff
33 children ages 6 to 8 years old with 3 staff

The records sample size consisted of 5 children?s records. Four (all) staff records for this site were reviewed at the Blocker YMCA administrative offices on 11/19/19.

Children were observed completing homework assignments, playing table top games and gross motor games in the gymnasium, and eating an afternoon snack which consisted of pretzels and fruit juice. Restroom procedures and table sanitizing were also observed. Medication administration was not observed. The center had four medications stored on site which were reviewed with regard to storage, labeling, written authorization and required documentation. Staff report that children are not routinely transported while in care and that there have been no accidents or injuries requiring first aid or medical attention since the last inspection.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.


Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-580-E-1
Description: Based on a review of records, the vendor was unable to demonstrate that each staff who work directly with children have Current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Current certification in first aid.
Evidence:
Four (all) staff records were reviewed.
The record for staff #4 whose date of hire was documented as 9/4/18, did not contain documentation of certification in cardiopulmonary resuscitation (CPR) .
The Senior Program Director confirmed that this staff is currently employed and was unable to provide documentation of this certification during the inspection.

Plan of Correction: The Senior Program Director stated that she will schedule staff #4 for certification in cardiopulmonary resuscitation (CPR) at the next available date with the goal of completing within 30 days.
