Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitor inspection was initiated on December 7, 2020 and concluded on December 11, 2020. The program director was contacted by telephone to initiate the virtual, remote inspection. There were 17 children present with three staff. The inspector reviewed a list of items required to complete the inspection. The inspector reviewed two children?s and three staff records during the virtual, remote inspection to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review and interview the center failed to ensure that each child has a physical examination by or under the direction of a physician within one month of the first day of attendance.
Evidence:
The record for child 1 does not contain documentation of physical exam.
a. Child has a documented date of enrollment as August 18, 2020.
2 - Administrator 1 could not provide a copy of the physical report when requested.

Standard #: 22VAC40-185-510-N
Description: Based on observation and interview the center failed to ensure that when an authorization for medication expires, the parent is notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.
Evidence:
1 - There is an Epi Pen in the center with an authorization that expired September 11, 2020. However, the medication remains in the center.
2 - Staff 2 confirmed that there was not an updated authorization in the child's record.

Description: Based on record review and interview the center failed to obtain a central registry finding within 30 days of employment or service.
Evidence:
