Thank you for your time and assistance with the unannounced monitoring inspection conducted January 8, 2020. The on-site inspection was held from 1:45 p.m. to 4:40 p.m. On this date, there were 75 children in seven classrooms and in the care of 15 staff members. Two additional staff members were available to assist where needed. The total enrollment for the program was 169 children. All seven classrooms were toured and observations were made in three of those rooms. There were eight staff members' records and eight children's records reviewed. Ten staff members and five children were interviewed. The medication and medication paperwork were reviewed for four children. Snack was observed to meet USDA guidelines and consisted of Fig Newtons and apple juice. An exit interview was conducted on this date. The playground was snow covered and was not observed. Some items observed and reviewed included: current health and fire inspections, required posted information, documentation of evacuation and shelter-in-place drills, warm water temperature, injury reports, fist-aid and emergency supplies, imaginative play, snow art activities, manipulatives, story time, movie time, diapering procedures, hand washing and bathroom procedures, nap time, afternoon snack, and dismissal procedures.

Standard #: 22VAC40-185-130-A Description: Based on review of eight children's records and verification from staff, the center did not obtain documentation of one child's immunization record showing the child was adequately immunized per age before the child's first date of attendance.
Evidence:
1. The record for Child #8 contained documentation of the child's immunization record that was dated 04/29/2019. First day of attendance for Child #8 was 04/22/2019.
2. Staff A confirmed Child #8's immunizations were received after the child had been at the center for a week.Plan of Correction: The center will ensure that an immunization record and documentation that a child is adequately immunized per the child's age is obtained before a child first day of attendance.
