Comments:
An unannounced monitoring inspection and two self-reported incidents were conducted on April 22, 2024, from12:45pm to 3:35pm.

During the investigations of self-reported incidents staff were interviewed, outside play space and infant classroom were reviewed. Additionally, face to name documentation, incident reports, a written statement and email documentation were reviewed. The self-reported incident regarding child elopement was found to be not valid as the child was never outside sight/sound supervision. The self-reported incident regarding an infant being fed another infant?s bottle is valid and a single violation regarding care and protection appears on the violation notice.

At the time of the monitoring inspection there were 65 children in care and nine staff members. Children were observed participating in play activities and nap. Five children and staff records were reviewed. Medication, evacuation drills, emergency supplies and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit meeting.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation and interview the center did not ensure that hazardous substances are kept in a locked place.
Evidence:
1.There were unlocked chemicals such as bleach and sanitizer in the toddler classroom.
a. The chemicals were in the lower storage cabinet of the diaper changing table and accessible to children.
b. There were 10 children in care at the time of the inspection.
2. The program director confirmed that the bleach and sanitizer were left unlocked and were accessible to the children in care.

Plan of Correction: The lower cabinet has a functioning lock. Teachers and management were not made aware after the installation of the lock feature, that if not careful, it could be disengaged if slid out of locking position.
On 4/22/2024, management provided the classroom with a child safety lock to ensure chemicals were locked safety and not accessible to the children until work order could be observed. Management will be sure to complete daily safety checklists to ensure cabinets are locked.

Standard #: 8VAC20-780-330-B
Description: Based on observation and interview the center did not ensure that resilient surfacing for
playground equipment complies with required safety standards.
Evidence:
1.The playground used by preschool through school-age children did not have at least six inches of resilient surface.
2.The program director confirmed that there was not six inches of resilient surface in the fall zones on this playground.

Plan of Correction: A work order was placed to replenish mulch on the playground to ensure sate fall zones while licensing was present on 4/22/2024. Director and DM will follow up with Facilities Management on status of mulch no later than 5/21.

Standard #: 8VAC20-780-340-A
Description: Based on staff interviews the center did not ensure that when staff are supervising children, they shall always ensure their care and protection.
Evidence:
1. On April 10, 2024, at approximately 8:30am, Staff #2 fed a seven month-old child, another child's bottle containing breast milk.
a. Staff #1 reported the incident to the department, on April 10, 2024.
2. Staff #2 stated that she fed child #1 the wrong breast milk bottle on the morning of April 10th. She stated that she reached in the refrigerator and removed a bottle without checking the label.
a. Additionally, Staff #2 stated that the bottle contained 8 ounces at the start of feeding. Child #1 drank an ounce when she realized it was the wrong bottle.
3. Staff #2 stated the infant bottle was not labeled.
a. Staff #1 stated that the label fell off during the warming bath.
4. Incident reports obtained from the center document that Child #1 was fed a breast milk bottle that belonged to another infant in care.

Plan of Correction: Teacher's have been retrained on making sure when transferring bottles to the refrigerator, that all bottles have appropriate labels. The Director enforced a bottle acknowledgement procedure that requires all teachers to verbally announce the child's name and bottle type, listed on label, to another teacher present in the room before providing the bottle. Directors also check the refrigerator upon arrival in the morning to ensure safety compliance.
