Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on July 16, 2020 and concluded on July 17, 2020. The director was contacted by telephone to initiate the inspection. There were 218 children present and 18 staff. The Inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of records on July 16, 2020, the center did not ensure to obtain documentation of a physical examination by or under the direction of a physician for each child before the child's attendance or within one month after attendance.
Evidence: 1. The record of Child #3 (DOE 06/15/2020) did not contain documentation of a physical exam.

Plan of Correction: We have already requested the physical from the parent.

Standard #: 22VAC40-185-260-B
Description: Based on a review of records on July 17, 2020, the provider did not ensure annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for: water supply, sewage disposal system, and food service, if applicable.
Evidence: The health inspection is dated 8/29/18.

Plan of Correction: Requested Health Inspection. They will notify us when inspections can start back up again.

Standard #: 22VAC40-185-560-F
Description: Based on a review of records on July 17, 2020, the provider did not ensure to follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence: 1. Documentation of the center's menu contains one component of the USDA's two component requirement.
2. Administration acknowledged the center was only serving one component for each snack provided.

Plan of Correction: Adding 1 additional component.
