Technical Assistance:
Medication procedures discussed. Suggest updating procedures for storing of medication for children in attendance in both the private school and licensed programs operating within the private school.

All licensed child day programs that serve preschool-age children operating in a building built before 2015 shall be equipped with at least one carbon monoxide detector. Written information emailed to PD.

Broken area of fence on the outdoor play area should be repaired to avoid a potential safety hazard in the future.

Discussed background check and training requirements for staff working in the preschool program.

Comments:
A modification visit was conducted on 01/31/2024 to review space as part of a request to add preschool age children to the current license. A monitoring inspection was conducted in conjunction with the modification inspection which began at 2:00 pm and concluded at 6:00pm on 01/31/2024.

There were 17 school age children in care with 3 staff and 11 preschool children in care with 3 staff.
Children were observed during outdoor play, afternoon snack and reading/quite time. Records were reviewed for six children and seven staff. Background checks were also reviewed for members of the school board.

Based on the information gathered violations were found in the areas of administration, staff training, physical plant and special care provisions & emergencies. These violations are listed on the violation notice issued to the facility and were reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review and interview, the licensee failed to ensure that documentation of the required tuberculosis shall have been completed within the last 30 calendar days of the date of employment and prior to coming into contact with children.

Evidence:
1. The staff listed below obtained the results of the required TB screening after their hire date:
a. Staff 1, hire date 10/19/23, obtained TB screening on 10/26/23;
b. Staff 2 , hire date 10/06/23, obtained TB screening on 12/05/23;
c. Staff 3 , hire date 11/20/23, obtained TB screening on 12/01/23.
2. Administrative staff confirmed that the TB screenings for these staff was obtained after employment.

Plan of Correction: Administrative staff that all new staff will obtain a TB screening prior to hire ( and within 30 days of hire) going forward.

Standard #: 8VAC20-780-240-A
Description: Based on record review and interview, the licensee failed to ensure that staff complete the Virginia Department of Education-sponsored orientation within 90 calendar days of employment.

Evidence:
1. Staff 1, hire date 10/19/23, has not completed Virginia Preservice training.
2. Staff 2, hire date 10/06/23, has not completed Virginia Preservice training.
3. Staff 4, hire date 04/20/23, has not completed Virginia Preservice training.
4. Administrative staff confirmed that these staff had not completed Virginia Preservice training.

Plan of Correction: Administrative staff stated that these 3 staff are currently working on Virginia Preservice training and anticipate completion within 30 days.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the licensee failed to ensure that areas of the center shall be maintained in a safe condition.

Evidence:
1. There is an area of drywall, in the preschool children's reading area, that has a sharp point at the corner near the floor.
a. This area is within easy reach of children sitting in the reading area.

Plan of Correction: Administrative staff stated the facility maintenance will be notified immediately of repair needed to this area of the wall in the preschool classroom.

Description: Based on record review and interview, the licensee failed to ensure that there was one-working, battery-operated radio in each building used by children.

Evidence:
1. The battery-operated radio for the 2nd floor preschool area lacked working batteries.
a. Administrative staff stated that working batteries could not be located for the radio.

Plan of Correction: Administrative staff stated that batteries will be obtained for the radio.

Description: Based on record review and interview, the licensee failed to ensure that shelter in place procedures shall be practiced a minimum of twice per year.

Evidence:
1. The emergency practice log for the facility did not indicate any shelter practice drills for 2023.
2. Administrative staff verified that no shelter practice drills had been completed in 2023 for the licensed program.

Plan of Correction: Administrative staff stated that two shelter drills will be completed in 2024.

Standard #: 8VAC20-780-550-I
Description: Based on observation, the licensee failed to ensure that 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone.

Evidence:
The phone numbers for 911 or local emergency numbers and poison control were not posted near the telephone in the preschool classroom.

Plan of Correction: Telephone numbers for poison control and 911 will be added to the wall by the telephone in the preschool classroom immediately.
