Comments:
A complaint inspection was initiated on 3/15/2024 and concluded on 3/15/2024. The inspector reviewed compliance in the areas of staffing and supervision, special care and emergencies and nutrition. The inspector reviewed video footage. The inspection started at 10:50 am and concluded at 12:30 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-A
Complaint related: Yes
Description: Based on video reviews, the center failed to ensure that when staff are supervising children, they shall always ensure their care, protection, and guidance.
Evidence:
Video footage on 3/11/24 shows child #1 getting a finger caught in the door that is adjacent to the toddler room. Staff #1 had the door open and was talking with someone in the toddler room. Video footage showed child #1 holding the left hand and crying. Staff #1 never acknowledged child #1 or consoled the child. Video footage showed staff #1 looking at a cell phone. Approximately two minutes later video footage showed staff #2 coming into the room to give staff #1 a break. Staff #1 notices blood and then sees the child?s finger was bleeding.

Plan of Correction: Staff has been given the Standard For Licensing regarding the violation. Staff has read the Standard and reviewed the standards and the video with administration.

Description: Based on video reviews, the failed to ensure that children under 10 years of age shall be within actual sight and sound supervision of staff.
Evidence:
Video footage from 3/11/24 shows staff #1 walking to the back of the room while child #1 was at the front of the room. Dividing the room is a solid diapering table, a cabinet, and shelves. Child #1 was not in actual sight supervision due to staff #1 being turned with back facing child and child #1 was often behind the diapering table, cabinet, and shelves. Approximately two times, staff #1 was sitting in a chair looking at the cell phone for more than one minute and not watching child #1

Description: Based on video reviews, the center failed to ensure that children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks. Staff shall wash their hands with soap and water before and after a diaper change and after any contact with body fluids.
Evidence:
Video footage from 3/11/24 shows that staff #1 did not wash child #1?s hands prior to eating snacks.
Evidence:
Video footage from 3/15/24 shows that staff #1 did not wash hands before or after changing a child?s diaper.
Based on video reviews, the center failed to ensure that staff shall wash their hands with soap and running water after any contact with body fluids.
Staff #1 blew her nose in a tissue and did not wash her hands.

Description: Based on video reviews, the center failed to ensure that the diapering surface shall be cleaned with soap and water and sanitized after each use.
Evidence:
Video footage from 3/15/24 shows staff #1 changing a soiled diaper. The surface was sprayed with soap and water and sanitizing agent at the same time and then wiped dry.

Description: Based on video reviews, the center failed to ensure that centers shall follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture. Children shall be allowed second helpings of food listed in the USDA?s child and adult care meal patterns.
Evidence:
Video footage on 3/11/24 shows staff #1 putting dry cereal in a bowl and placing it on the shelves for child #1 to eat. There should be at least two of the five food components offered to children for snack.
Video footage also shows child #1 spilling the bowl and staff #1 sweeps up the cereal and does not offer seconds to the child.

Complaint related: Yes
Description: Based on video reviews, the center failed to ensure that no child shall be allowed to drink or eat while walking around.
Video footage on 3/11/24 shows child #1 getting a dry bowl of cereal off a shelf and walking around the room eating. Child # 1 ended up walking to the corner of the room and spilled all their cereal on the floor when trying to sit.

Plan of Correction: Staff has been given the Standard For Licensing regarding the violation. Staff has read the Standard and reviewed the standards and the video with administration.
