Comments:
The Licensing Inspector for the Roanoke YMCA After School Program @ Virginia Heights Elementary School conducted an unannounced Renewal Inspection on Friday, December 13, 2019. Upon arrival there were a total of twelve (12) children ages 5-10 years old present under the direct care & supervision of three (3) staff members. Five (5) children records were reviewed during today?s inspection. Staff records are maintained at the Roanoke YMCA home office and were reviewed on 12/18/2019. Licensing Inspector observed children during departure procedures, homework assistance, meals, outdoor activities, bathroom/hand washing procedures, group activities and self-initiated activities.

Violations found were reviewed during the exit interview with the Site Director on 12/13/2019 and the Program Director on 12/18/2019. Violations:
Standard #: 22VAC40-185-540-E
Description: Based on inspection, the Licensee failed to ensure non medical emergency supplies are operable.

EVIDENCE:
The battery operated radio was inoperable.

Plan of Correction: Will replace radio and purchase additional batteries.

Standard #: 22VAC40-185-550-C
Description: Based on inspection, the Licensee failed to ensure evacuation and shelter in place procedures and maps are posted in a location conspicuous to staff and children on each floor of each building.

EVIDENCE:
The center's emergency maps and procedures were not posted on the 2nd floor of the school building. The 2nd floor cafeteria is used to provide care for the children.

Plan of Correction: Emergency maps and procedures will be posted on each floor used by the children in care.
