Comments:
A Subsidy Health and Safety inspection was conducted on December 27, 2019 to review supplemental health and safety requirements. The inspection was initiated at 11:00 am and concluded at 1:20 pm. Today the center offered full day care as the public schools were closed for winter break. Children, second through third grade, were observed playing outside in the gaga pit and tossing a football with staff. Kindergarten and first graders played in centers while the fourth and fifth graders were on a field trip at the Science Museum. Later children played video games, completed art projects and played a group game in the gym. Lunch was observed with children providing their own lunch. The center is equipped with age appropriate materials and supplies. The first aid kit and non-medical emergency supplies were inspected and found complete. Required postings were reviewed. The inspector interacted with staff and children in care.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Six children?s records were reviewed along with emergency contact information for nine staff. Medication is being administered and was reviewed. Staff on site have current CPR, First aid, DHO and MAT certification.
The most recent evacuation drill was documented on 12/19/2019. The two required shelter-in-place drills were completed on 02/04/2019 and 10/02/2019.

Today's census: 56 school age children with 9 staff.

Description: Based on review of children's records, the center did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:

The record of child #10, with a diagnosed food allergy, did not contain a written allergy care plan.

Plan of Correction: Per the director: I will contact the parent to obtain an allergy care plan.
