Technical Assistance:
Discussed in detail the requirements (8VAC20-780-330) related to playground equipment and resilient surfacing that shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.

Comments:
An unannounced monitoring inspection was started in the center on April 23, 2024 from 10:20 AM until 11:45 AM and concluded on April 23, 2024. At the time of entrance 34 children were in care with 4 staff members. The sample size consisted of 5 children?s records and 5 staff records. Children were observed during music class playing in small groups to include manipulative activities and dramatic play.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-40-M
Description: Based on observation and staff interviews, the licensee did not ensure the center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.

Evidence: Child #6 has a food sensitivity, (mushrooms) and it was not listed on the children's food allergies, sensitivities, and dietary restrictions list in the classroom where Child #6 was observed. Center staff and documentation confirmed the food sensitivity for Child #6.

Standard #: 8VAC20-780-80-A
Description: Based on documentation review and provider interview the licensee did not ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: There were 16 children observed in the classroom and Staff #3 confirmed there was not an attendance sheet that marked the arrival of children as it occurred.

Standard #: 8VAC20-780-510-L
Description: Based on observation and staff interviews, the licensee did not ensure, medication, except for those prescriptions designated otherwise by written physician's order, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: In room #525 there were 5 medications stored in a backpack on a top shelf inside a unlocked cabinet. These medications were not stored in a locked place.
