Comments:
Thank you for your assistance and your time during this unannounced monitoring inspection. The visit was conducted on November 22, 2019 from 3:30 p.m. through 5:40 p.m. There were twenty-eight school-age children in the care of three staff. The total enrollment for the program is thirty-two children. There were five children's records and three staff records reviewed and updated. The medication and authorization paperwork were reviewed for three children. The exception to compliance was noted on the violation notice. Snack was observed and met USDA food guidelines and consisted of graham crackers, peanut butter, and milk. Hand washing, free choice, tabletop toys, card games, organized play in the gym, outside play, and dismissal procedures were observed during this visit.

Standard #: 22VAC40-185-240-C
Description: Based on review of three staff members' records and verification from the administrator, one staff member failed to obtain 16 hours of required annual staff training.

Evidence:
1. Staff member #3 obtained 10 annual training hours of the 16 required for 2018-2019. Staff member #3's date of hire was 07/06/2015.
2. Administrator confirmed that Staff member #3 did not have the correct required number of staff annual training hours.

Plan of Correction: The staff member will obtain the rest of the required training hours as soon as possible.
