The licensing inspector conducted an unannounced monitoring inspection on 10/28/2019 from 1:55pm to 5:37pm. The children were observed learning through play in centers, putting together puzzles, participating in story time and playing on the playground. A formal observation was completed with the Preschool Explorers and Stepping Stones classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today.
Ten children?s records and ten employee records were reviewed during this inspection. Medication is being administered and was reviewed. Seven injury reports were reviewed..
The center's first aid kit and emergency supplies were inspected.

Standard #: 22VAC40-185-520-C Description: Based on observation and an interview, the center did not ensure that when diaper cream is used, all of the requirements were met.
Evidence:
1. The center was not able to provide documentation of parent permission for child #11's diaper cream.
2. The center's staff stated that child #11's diaper cream was used on the child and the center did not have a parent permission form for the diaper cream.Plan of Correction: Not available online. Contact Inspector for more information.
