Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on December 17, 2019 from 10:50 a.m. 2:00 p.m. There were eighty-two children in the care of nineteen staff in eight classrooms. The medication and medication paperwork was reviewed for two children. Eight children's and ten staff records were reviewed and updated. Areas of non-compliance may be found on the violation notice.
Areas observed were: free choice activities, circle time, hand washing/bathroom and diapering procedures, lunch, transition and nap. Interactions between the children and staff were observed as positive and the children seemed comfortable with their daily routine and schedule.

Standard #: 22VAC40-185-500-A Description: Based on observation, children's hands were not washed, after diapering, in three classrooms.
Evidence:
1. Diapering was observed in three classrooms.
2. Staff #1, #2, and #3 were observed not washing children's hands after being diapered.
3. Staff #1, #2, and #3 confirmed they did not wash children's hands after being diapered.
Based on observation, children did not wash hands before eating lunch in one classroom.
Evidence:
1. Classroom #3 was observed getting ready for lunch.
2. The children sat down at the tables to eat lunch and were observed not washing hands.
3. Staff confirmed children did not wash hands before eating lunch.Plan of Correction: Staff will be retrained on all hand washing procedures.
Standard #: 22VAC40-185-500-B Description: Based on observation, the center failed to have a nonabsorbent changing pad in Classroom #1.
Evidence:
1. The diaper pad in Classroom #1 was observed with tears, which exposed the foam material.
2. The staff confirmed the diaper pad was torn.Plan of Correction: A new diaper pad will be purchased immediately.
