- sworn statement or affirmation - must be completed prior to employment and updated every five years
- fingerprinting - must be submitted prior to employment and updated every five years
- search of the central registry - must be submitted within seven days of employment and received within 30 days of employment and updated every five years.

Comments:
A non-mandated monitoring (code compliance) inspection was conducted on 11/21/2022. The director was contacted by telephone to initiate the inspection. There are 26 children enrolled and 11 employed staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed three staff records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on information for three staff that was submitted by the religiously exempt facility, it was determined that the facility did not ensure that all staff completed fingerprinting prior to employment.

Evidence:
The information submitted for staff #3 (who stated during the inspection that she had been with the facility for six years) did not include documentation of completed fingerprints.

Plan of Correction: Staff #3 completed fingerprints on 11/22/2022. Results will be forwarded to licensing inspector when received.

Standard #: 8VAC20-770-40-D-2
Description: Based on information submitted for three staff records, it was determined that the religiously exempt facility did not ensure that all staff had had a completed sworn statement or affirmation on file before their first day of employment and that all staff had a completed search of the central registry on file within 30 days of employment.

Evidence:
1. The information submitted for staff #1 did not include a sworn statement or affirmation and a completed search of the central registry. Staff #3 (program director) stated during the inspection that staff #1 had been working "since summer."
2.The information submitted for staff #2 did not include a sworn statement or affirmation and a completed search of the central registry. Staff #3 (program director) stated during the inspection that staff #1 had been working "since summer."
3.The information submitted for staff #3 did not include a sworn statement or affirmation and a completed search of the central registry. Staff #3 (program director) stated that she had been working at the facility for about six years.

Plan of Correction: Facility human resources department will ensure that all staff have completed searches of the central registry on file within 30 days of employment. Once these checks are received for staff #1, #2 and #3, the results will be forwarded to licensing inspector.
