Comments:
The Licensing Inspector conducted an unannounced Subsidy Health and Safety inspection on Thursday, October 6, 2022 from approximately 1:15pm to 2:50pm. There were nine children in attendance and a total of two staff supervising. A total of five children?s files and two staff files were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-540-B
Description: Based on review of five children's records, the vendor did not ensure five children's records contained the required information. Evidence: 1. The records for Child #1, Child #2, Child #3, Child #4 and Child #5 did not contain the address for the listed emergency contacts. 2. Staff #2 confirmed this information during the inspection.

Description: Based on review of five children's records and interview, the vendor did not ensure within the first 90 days of employment one staff who worked directly with children completed Virginia Preservice Training for Child Care Staff. Evidence: The record for Staff #1 (Date of Employment 07/12/2021) did not contain documentation of completing Virginia Preservice Training for Child Care Staff. Staff #2 stated Staff #1 had not completed the training as of 10/06/2022.

Description: Based on review of two staff records and interview, the vendor did not ensure one staff member who worked directly with children annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course. Evidence: 1. Staff #1 (Date of Employment 07/12/2021) did not contain documentation of completing at least 16 hours of annual training. Staff #2 stated Staff #1 had not completed the required annual training as of 10/06/2022.

Standard #: 8VAC20-790-640-A
Description: Based on observation, the vendor did not ensure all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevented access by children. Evidence: 1. Two containers of sanitizing wipes were sitting in two classrooms, accessible to the children. 2. One container of sanitizing wipes was sitting on the shelf in the bathroom.

Description: Based on documentation review and interview, the vendor did not ensure the emergency response drills were practiced as required. Evidence: The documentation of emergency drills did not document a September 2022 evacuation drill. Staff #2 stated she forgot to complete an evacuation drill for September 2022.
