Comments:

An unannounced monitoring inspection was conducted today from 11:00am-2:00pm. There were 55 children directly supervised by 12 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 3 staff records, 6 children's records, 7 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed coloring, engaging in table top activities, playing indoor soccer, doing crafts, and playing alphabet bingo. Infant care and feeding was observed. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 5pm on Friday, January 10, 2020. Standard #:	22VAC40-185-130-A
Description:	Based on record review, 1 of 6 children's records did not contain documentation that the child has received the immunizations required by the State Board of Health before the child can begin attending the center. Evidence: Child #6 (Start Date: 5/24/2019) does not have the immunization record on file.
Plan of Correction:	An immunization record was obtained on 1/3/2020.
Standard #:	22VAC40-185-140-A
Description:	Based on record review, 1 of 6 children's records reviewed did not contain the results of a physical examination obtained within one month after child's first day of attendance. Evidence: Child #6 (Start Date: 5/25/2019) does not have a physical examination on file.
Plan of Correction:	A physical examination was obtained on 1/3/2020.
Standard #:	22VAC40-185-160-C
Description:	Based on record review, 1 of 3 staff records did not contain documentation of an updated tuberculosis screening or test completed within the last 2 years. Evidence: Staff #1's most recent Tuberculosis test was completed 1/26/2017.
Plan of Correction:	A current record of the staff's tuberculosis has been obtained. Corrected on 1/4/2020.
Standard #:	22VAC40-185-510-A
Description:	Based on review of 7 medications and medication authorization forms, it was determined that 3 children had medications on site without signed parental authorization to administer the medications. Evidence: Child #7, Child #8, and Child #9 had medications on site and did not have signed parental authorizations granting the center permission to administer the children's medications.
Plan of Correction:	All forms needing parental signature were signed on 1/3/2020. The additional medical forms on file did have physician signature.
Standard #:	22VAC40-191-60-C-2
Description:	Based on record review, 1 of 3 staff records reviewed did not contain the results of a central registry search within the first 30 days of employment. Evidence: Staff #1 (Date of Hire: 8/29/2017) does not have the central registry results on file.
Plan of Correction:	The school does a FBI Finger Print Verfication completed on file for this staff member. A new Central Registry Search was mailed on 1/3/2020.
