Comments:
An unannounced monitoring inspection was completed on August 30, 2019 from approximately 10:00 a.m. to 12:00 p.m. There were no children in care during the inspection due to a staff work day but staff were present on the premises. Staff were observed in classrooms preparing for the care of children. Interviews and interactions were conducted and medications were reviewed. Ten children?s records and five staff records were reviewed. The Director and a Co-Director were available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #: 22VAC40-185-60-A
Description: Based on a review of children?s records, the licensee failed to maintain all the elements of a child?s record for one of ten children.
Evidence: 1. Child #6's record, enrolled on May 13, 2019, lacked emergency contact addresses. 2. Administrator #1 acknowledged this information was not documented in the record.

Plan of Correction: The Director will obtain this information from the child's parent.

Standard #: 22VAC40-185-550-M
Description: Based on a review of records, the licensee failed to maintain all required elements of a written record of children's serious and minor injuries in which entries are made the day of occurrence.
Evidence: 1. Injury reports from June, 2019 to August, 2019 were reviewed. Approximately half of the reports lacked the time the parent was notified of the injury as required. 2. Administrator #1 acknowledged this information was missing from these reports.

Plan of Correction: The Director will send written notice to all staff to remind them of this requirement and will check the reports as they are turned in for all the required information.
