Comments:
An unannounced monitoring inspection was conducted today. There were 95 children in care with 15 staff and administrators. Children in care ranged in age from five months to 10 years old. Children and staff records were reviewed. Children were observed in teacher and self-directed activities including play activities, transitions, lunch, hand washing, restroom routines and nap time. Required postings, annual inspections, first aid kit and emergency items were reviewed. Violations regarding food brought from home and resilient surface were identified and appear on the violation notice. These violations were reviewed with the program director at the conclusion of the inspection.

Standard #: 22VAC40-185-40-K
Description: Based on observation and interview the center failed develop written playground safety procedures that include resilient surface maintenance. Evidence: 1 - When asked to review the center's written playground safety procedure including the resilient surface maintenance plan, staff 1 stated that the center did not have a written resilient surface maintenance plan. 2 - Staff 1 stated that the resilient surface is replenished once a year.

Plan of Correction: Play ground safety and maintenance plan written 8/2/19 a copy was given to you at our suffolk center.

Standard #: 22VAC40-185-330-B
Description: Based on observation and interview the center failed to ensure that where playground equipment is provided, resilient surfacing, fall and use zones are in compliance with safety standards. Evidence: 1 - The resilient surface depth on the playground measures between 1/2 inch and three inches throughout the playground including the fall and use zones of climb and slide equipment where six inches is required. 2 - Staff 1 confirmed that there was less than six inches of resilient surface on the playground.

Plan of Correction: resilient surface has be delivered and will be put down by 8/12/19

Standard #: 22VAC40-185-560-G
Description: Based on observation and interview the center failed to ensure that when food is brought from home it is labeled with date. Evidence: 1 - Two lunch boxes in the school age room do have dates on them. 2 - Staff 1 confirmed that there were two lunch boxes that do not have the date on them.

Plan of Correction: any child bringing a lunch box has it labeled and dated when entering the classroom 7/26/19
