Comments:
An unannounced monitoring inspection was conducted today from 11:20 am to 1:50 pm with the director. Eighty one children were present with 13 staff supervising. The children present ranged in age from 2 months to 5 years.
For lunch, the children were served fish tacos, corn, peaches and milk. One child had medication on site. One allergy care plan was reviewed as well as 5 injury/accident reports. The two vans used for transportation were also inspected today. Three staff and six children's files were reviewed. The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-40-M
Description: A current written list of all children with allergies, sensitivities and dietary restrictions shall be in each room where children are present.
Child D's allergy to eggs were not documented on the written list.

Plan of Correction: We will add the allergy to the written list. Allergy list was amended to show the children's allergy, parent's preference and substitute diet restrictions due to religious reasons.

Standard #: 8VAC20-780-60-A-8
Description: The center must obtain a written allergy care plan for each child with a diagnosed food allergy.
There was no allergy care plan on file for child A to include signs and symptoms to look for to determine exposure. Child A is allergic to milk and nuts.
There was no allergy care plan on file for child B who is allergic to eggs and milk
There was no allergy care plan on file for child C who is allergic to sesame, eggs, soy and milk.
There was no allergy care plan on file for child D who is allergic to eggs.

Plan of Correction: We will obtain the allergy care plans from the physicians. We have spoken to the parents about their child's allergy plan to ensure they include all allergies including substitute diet restrictions. On one form or multiple allergy care plan forms.

Standard #: 8VAC20-780-280-B
Description: Repeat violation:
Hazardous substances shall be kept locked.
There were carpet cleaner and adhesive remover unlocked in the storage room at the font of the center.

Plan of Correction: The closet was locked during the inspection. The storage closet was reorganized so the closet arm will close properly.
