Standard #: 8VAC20-780-270-A Description: Based on observation, the center failed to ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: Playground #1, on the door of the metal gate surrounding the playground, the door as come unattached at the hinge. This creates a gap in the gate causing an entrapment hazard to children. Children were observed playing on the playground during the inspection.
Playground #2: The sandbox on the playground has mesh lining that is coming through the surface of the sandbox causing a tripping hazard to children.Plan of Correction: Playground #1 The gate has been ordered and will be installed as soon as it comes in. 11-18
