Standard #: 22VAC40-185-130-B Description: Based on a review of nine children's records, one record did not contain documentation of additional immunizations every six months for children under the age of two. Evidence: The record for Child #9 (start date 7/9/18) did not contain an updated immunization record. Plan of Correction: The facility will request the documentation from the child's parents.
Standard #: 22VAC40-185-500-B Description: Based on observations, the facility did not ensure that a nonabsorbent surface for diapering or changing was used. EVIDENCE: The inspector observed tears in the non absorbent surface covering on the pads used to change diapers in the younger infants room, the older infants room, and the two year old classroom. The tears exposed fabric that was not nonabsorbent. Plan of Correction: The changing pads were removed in each classroom during the inspection. The facility will use the nonabsorbent surface of the changing tables for diapering.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on a review of nine staff records, two records did not contain an Out of State Child Abuse and Neglect Search Request. Evidence: 1. The updated Sworn Disclosure Statement indicated Staff #4 (hire date 2/18/19) lived out of state within the past 5 years and the record did not contain an Out of State Child Abuse and Neglect search request. 2. The updated Sworn Disclosure Statement indicated Staff #5 (hire date 12/11/18) lived out of state within the past 5 years and the record did not contain an Out of State Child Abuse and Neglect search request. Plan of Correction: The facility will ensure the out-of-state search requests are submitted and timeframes for background checks are closely monitored.
