An unannounced monitoring inspection was conducted on 5/30/19 from 10:20am - 1:00pm. During the inspection there were 129 children ages two months old through six years old in care with 30 staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch, and during rest period. Records were reviewed for nine children and ten staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Standard #: 63.2(17)-1720.1-B-3 Description: Based on a review of ten staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 45 days of employment from any other state in which the employee has resided in the preceding five years. Evidence: 1. The record for staff #8 (date of hire 10/12/18) did not contain documentation of a completed search of the central registry finding. 2. Staff #9 (Program Director) review the record for the staff #8, and confirmed that the search of the central registry finding from the state in which staff #8 previously resided in has not been received. Plan of Correction: The facility responded: The search of the central registry for staff #8 was sent off previously by another staff member. We called to see if it had been received,but they said it had not, so staff #8 will complete a new search of the central registry for that state. All new staff who have resided in another staff within the last five years will complete a search of the central registry for the corresponding state.
