Comments:
An unannounced monitoring inspection was conducted on 7/25/19 from 9:58am - 11:35am. During the inspection there were 35 children ages two years old through five years old in care with five staff. Children were observed participating in various activities in the classrooms, and playing outside. Records were reviewed for five children and five staff. There was no medication at the center. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence 1. The record for child #1 did not indicate whether or not the child had previously attended child care. 2. The record for child #2 did not indicate whether or not the child had previously attended child care. 3. Staff #1confirmed that the records for child #1 and child #2 did not contain all of the required information.

Plan of Correction: The facility responded: the parents of child #1 and #2 will be asked to complete the missing information. all newly enrolled children will have the records reviewed prior to enrollment

Standard #: 22VAC40-185-270-A
Description: Based on observation and interviews, it was determined that the facility did not ensure that all areas and equipment of the center shall be maintained in a safe and operable condition. Evidence: 1. There are broken boards on the wooden exterior fence on the large playground. On two boards there are exposed nails. 2. On the small playground there is a section of the fence by the dumpster that is bowing which has exposed some nails.. 3. Staff #1 confirmed the items listed above were not in a safe and operable condition.

Plan of Correction: The facility responded: A work order will be placed to have both fences repaired. Staff will continue to monitor and inspect all areas of the center to ensure they are maintained in a safe and operable condition.

Standard #: 22VAC40-185-540-E
Description: Based on a review of the emergency supplies at the center, it was determined that the licensee did not ensure that there is one working battery-operated radio in each building used by children. Evidence: 1. There was not a working battery-operated radio available during the inspection. 2. Staff #1 could not find batteries during the inspection to demonstrate that the battery-operated radio was working.

Plan of Correction: The facility responded: We will purchase additional batteries to ensure the radio will be operation during an emergency.
