An unannounced monitoring inspection was conducted on 1/14/20 from 9:10am - 10:35am. During the inspection there were 23 children ages two years old through five years old in care with four staff. Children were observed participating in various activities in the classrooms, and eating morning snack. Records were reviewed for five children and five staff. There was no medication at the center. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Standard #: 22VAC40-185-160-C Description: Based on a review of five staff records, it was determined that the facility did not ensure that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
1. The record for staff #3, contained documentation of TB screening that was dated 8/25/17.
2. Staff #1 reviewed her record, and confirmed that she had not received an updated TB screening for staff #3.Plan of Correction: The facility responded: Staff #3 will complete an updated TB screening. All current staff will complete an updated TB screening every two years from the previous TB screening.
