A monitoring inspection was conducted on 6/2/2023 with the licensing inspector on site from approximately 3:30pm to 5:40pm. The center director and center staff were available throughout the inspection to answer any questions. There were 19 children present with 4 staff supervising. Interviews were conducted with staff and areas of the facility were inspected to include all classrooms, bathrooms and the playground areas. Additional, documentation was reviewed on 6/5/2023. A total of 4 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

This list shall be dated and kept confidential in each room or area where children are present.
