Comments:
An unannounced renewal inspection was initiated on October 16, 2023. The inspector was onsite from 3:05pm to 5:00pm on October 16, 2023. Staff records were provided for remote review on 10/17/2023 and 10/18/2023.

There were 13 children present, ranging in ages 5 to 11 years, with 3 staff supervising. A total of five child records and four staff records were reviewed. Background checks for the board officers remain in compliance. During the inspection, the children and staff were observed participating in a variety of activities to include outdoor activities. Staff were observed having positive interactions with the children.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable.

Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Based on review of 4 staff records, the center failed to ensure The Virginia Department of Education-sponsored orientation course was completed by all staff within 90 calendar days of employment.

Evidence:
1.The record for Staff #3 (date of employment: 6/8/2023) did not document completion of The Virginia Department of Education-sponsored orientation course.
2.A center representative confirmed via an email provided on 10/18/2023 that Staff #3 was ?missing? The Virginia Department of Education-sponsored orientation course.

Description: Based on review of documentation and interview, the center did not provide to the licensing representative an annual fire inspection report.

Evidence: The most recent fire inspection report on record was dated 6/28/2022.

Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.

Evidence:
1.A crack of approximately 24 inches in length was observed in one of the windows of the building used for childcare, creating sharp edges that could cause injury to a child in care. A child was observed participating in an activity directly in front of the window.
2.The two bathrooms located within the building used for childcare had visible dirt build up throughout the floors. Staff #1 acknowledged the bathrooms needed cleaning.
3.The top of the baseboard heating unit in both bathrooms used by children in care had rust stains.
4.The sink fixture in one of the bathrooms used by children was loose from the sink.
5.A tile piece was missing from one of the bathroom walls used by children.
6.The building used for childcare had a torn carpet area within the path where children walk.
